Insurance

Insurance companies we accept are PPO plans only:

  • Medicare

  • Anthem Blue Cross Blue Shield

  • Cigna

  • Aetna

  • Humana (PPO Only)

  • United HealthCare

  • HealthNet

  • Medicare Advantage Plans (PPO Only)

We do not accept any HMO plans. Only vision plan we accept is: Vision Service Plan (VSP)

For all HMO patients requesting to pay cash for services, this will be for ONLY services NOT provided under their plan. Cash payment is due at time of service and patient will have to seek reimbursement from plan on their own.
